Genesis Healthcare Social Services Assistant in Mendham, New Jersey

The Social Services Assistant performs administrative support activities required for delivery of social services from patient admission through discharge.

Position Highlights

*Participate in the development of a written, interdisciplinary plan of care for patients/residents that identifies their psychosocial needs/issues, the goals to be accomplished for those needs/issues, and the appropriate Social Services interventions. *Assist patients/residents in coping with their transition and adjustment to a nursing center including the social, emotional, and psychological needs. * Educate patients/residents and families/significant others regarding their rights and responsibilities, health care decision making/advance directives, effective problem solving and the extent of community, health and social services that is available to them, including those necessary for effective discharge planning.

Qualifications: *Associate degree with a minimum of two (2) years' experience in long term care or human services. Note that these guidelines may vary according to State regulations. *Must have any certification/licensure required by state regulations. *Knowledge of casework processes, needs of geriatric customers, medical settings, and working knowledge of funding sources such as Medicare, Medicaid, and other insurances is required.
